I think the above is not right. In my opinion the meaning is simply a "consultation" I cannot see how anyone would elucidate from the phrase in question that it refers to a "terminally ill" patient?

I think this is a "clarifying speech with a patient, through documents". it is not only used for terminally ill patients, but also for patient who need surgery, f.e. for a prosthesis...
